Medical Marijuana (MMJ) certification and re-certification via a telehealth visit. Use your computer, tablet or phone to have a video meeting with Dr. Kudler. Fill out your medical history questionnaire and informed consent online for his review. If your record supports your request for certification, you will be directed to schedule a video appointment. If your record does not include a medical justification, he will let you know at no cost to you.
Fees:
Base rate: $100
Discount rate: $60 (must show proof of Medicaid, SNAP, WIC, FPHA, SSDI, veteran status)
10% discount: Colleagues in the cannabis industry, school teachers, and mental/behavioral health professions
